STEP 2 OF 2 - ENROLLMENT KIT NUMBER AND INFORMATION
Voya's Enrollment by Internet process provides high levels of privacy and security through a secured browser. Privacy Notice link provides information about the steps Voya has taken to safeguard your account information.
Please make note of your Kit Number. You will be required to enter this number to proceed with the online enrollment process.

Enrollment Process: Once the enrollment process is complete, you will have the chance to review and confirm your entries before submitting your enrollment information to Voya. Written confirmation of your enrollment will be mailed to your address on file. The confirmation will include a default Personal Identification Number (PIN). We recommend that you change your PIN once you receive this confirmation.
Payroll Authorization and Beneficiary Designation: In addition, to completing the online enrollment process, your employer may require that you complete a participation agreement form and/or beneficiary designation form.
IMPORTANT! You will not be enrolled until you complete all of the information required and click on the “Submit Enrollment” button at the end of this enrollment.

Please Note! You must enroll in each plan separately. After enrolling in one plan, you must return and complete these same steps and enroll in the second plan.

Group annuities offered through a retirement plan are long-term investments designed for retirement purposes. Early withdrawals from a 401(a) plan prior to age 59 1/2 are subject to an IRS 10% premature penalty tax, unless an IRS exception applies. Money distributed will be taxed as ordinary income in the year the money is distributed. Account values fluctuate with market conditions and, when surrendered, the principal may be worth more or less than the original amount invested. An annuity does not provide additional tax deferral, as tax-deferral is provided by the plan. Annuities may be subject to additional fees and expenses to which other tax-qualified funding vehicles may not be subject. However, annuities provide features and benefits such as lifetime income payments and death benefits, which may be valuable to you.
